A DETAILED DESCRIPTION OF A PHYSICIAN MEDICAL RECORDS REVIEW

Our Physicians’ Review of medical records focuses on the specialties and physicians that the client selects. They can be hospital based, ER Clinic practitioners, private office, or any combination. This includes, but is not limited to: Emergency Room, Anesthesia, Surgery, Family Practice, and all other specialties. The Review will focus on compliance with government guidelines, and accurate reimbursement. The Review includes ICD-9-CM Diagnosis codes, CPT Procedure codes, Evaluation and Management (E&M) codes, #1500 Billing Forms, Detail Bills, the proper use of Modifiers, and the appropriate level of Documentation. All errors and discrepancies identified will be thoroughly discussed, and recommendations for change will be made. A comprehensive, written report will be submitted after the end of the on-site visit, in a timely manner.
