  ICD-10 Coding and Clinical Documentation Improvement Preparation Project  
 
 
 

PRC has more than thirty (30) years of experience providing the highest quality of coding, compliance, and reimbursement related consulting services available to healthcare providers throughout the country.  Since PRC’s inception in 1979, we have conducted more than 2,000 individual projects, focusing on coding, documentation, compliance, and reimbursement. Throughout the years, PRC has assisted clients through the difficulties associated with rapidly changing healthcare regulations, and we have specifically designed an ICD-10 preparation project to help navigate facilities to a successful ICD-10 implementation in October 2013.              

The project includes an initial assessment of the Hospital’s current Clinical Documentation and Coding.  PRC will perform an analysis of historical claims data and frequency reports to assist in the selection of the appropriate records sample.  PRC will then review the selected sample of records that were coded and billed with the current ICD-9-CM classification system and report on our findings.  PRC will then code selected records using the ICD-10 system.  PRC will focus on the documentation of diagnostic and procedural statements to determine how well that documentation would support the new ICD-10 codes.  This initial review will help to identify specific documentation concerns that need to be improved, in order to ensure a more efficient conversion to the ICD-10 system.

During the review, educational sessions will be conducted with the Health Information Management Department and other specialty groups such as Clinical Documentation Improvement staff and Physicians as appropriate.  PRC will provide baseline education regarding the new classification system as well as facility specific education based on the review findings.  PRC will provide a comprehensive detailed report identifying documentation areas that the hospital should address in order to successfully convert to the new ICD-10 system.
